The South Korean ratings board, GRAC, has classified Death Stranding 2: On the Beach with an age rating of “19+” due to its content featuring "excessive violence," "excessive profanity, profanity and swearing," and scenes depicting the use of various illegal substances.

Hideo Kojima recently expressed his gratitude to actress Shiori Kutsuna for her contributions to Death Stranding 2. Kutsuna completed her work on the game, which included motion capture in Los Angeles, 3D scanning, and voice recordings in both Japanese and English across various locations, despite the challenges of the ongoing pandemic. Kojima's heartfelt message read, "For Shiori Kutsuna, that's it. I appreciate all of your hard work. Fantastic work, much obliged!" Kutsuna has been involved with the project since the fall of 2022.
At the Tokyo Game Show 2024, Hideo Kojima provided a glimpse into Death Stranding 2: On the Beach by showcasing several two-minute cutscenes. These scenes introduced viewers to key characters and their roles within the game's narrative. George Miller's character, Tarman, captains the mobile base Magellan, which navigates tar lakes. Fatih Akin portrays Dollman, a former medium who has transformed into a living doll. Léa Seydoux reprises her role as Fragile.
Elle Fanning's mysterious character, named Tomorrow, was found in a location reminiscent of the realm of the dead, tying into the original game's theme encapsulated by the tagline, "Tomorrow is in your hands."
One particularly engaging cutscene features characters singing "Raindrops Keep Fallin' on My Head," a classic tune by Burt Bacharach and Hal David, while interacting with a pregnant woman named Rainy, portrayed by Shiori Kutsuna.
